Please call our office at (330) 835-4000 to schedule an appointment, or visit our patient portal to schedule online. To ensure your appointment runs smoothly, please come prepared with your photo id, insurance card, list of current medications, relevant medical or surgical history, and any important questions you may have for your doctor.


Cancellation Policy

If for any reason you are unable to keep your appointment please notify us 24 hours prior to your scheduled visit. If you are unable to cancel or reschedule your appointment within 24 hours you will be charged a $50 cancellation fee. Please contact us here, or call our office for immediate assistance. Financial Notice

Co-pays and deductibles are due upon arrival for your appointment. There will be a $40 charge for all returned checks, including non-sufficient funds. There will be a $25 charge for each form brought in for completion. Payment is due when the form is presented. If you are late for your appointment, we may have to reschedule your appointment. If you are unable to to keep your appointment, please reschedule 24 hours prior to your appointment time, otherwise you will be charged a $50 cancellation fee.


Patient Safety Notice

Our office is a smoke/tobacco free environment.

Cell phone usage is prohibited during your appointment.

It is illegal to carry a firearm, deadly weapon, or dangerous ordnance anywhere on these premises. Unless otherwise authorized by law, no person shall knowingly possess, have under the person's control, convey, or attempt to convey a deadly weapon, or dangerous ordnance onto these premises. Posted pursuant to the Ohio Revised Code.


Insurance

Insurance is accepted at Signature Psychiatry Associates. Please bring your current insurance card to your visit so that can verify billing information. Co-pays are due at the time of your visit.

Signature Psychiatry Associates accepts cash, checks, Visa, MasterCard, Discover, and American Express.
